Artificial intelligence is spreading quickly across healthcare, but recent developments underscore how uneven the transition remains. In the past several weeks, new signals have emerged on three fronts: regulators are reshaping leadership and approach to digital health, federal agencies are increasing funding for AI-enabled imaging research, and health systems and insurers are trying to convert pilot projects into operational tools—often running into governance, payment and implementation hurdles.

FDA brings AI-sector leadership into digital health oversight

In a move that highlights the growing importance of software and machine learning in medical products, the U.S. Food and Drug Administration has selected an executive with an AI background to lead its digital health center, according to

. The appointment places a leader with private-sector AI experience at the center of the agency’s work on digital health tools, including software-driven products that may incorporate machine learning.

The leadership change comes as regulators globally face a difficult balancing act: enabling innovation while ensuring safety and effectiveness for tools that can evolve rapidly. While the FDA has spent years building frameworks for software and digital medical devices, the arrival of an AI executive in a key role reflects the extent to which AI is now central to the agency’s remit, not a niche topic.

Health systems report “execution paralysis” as pilots outpace deployment

Even as AI tools proliferate, many hospital and health system leaders say they are struggling to implement them at scale. A study highlighted by

describes “AI execution paralysis,” a pattern in which organizations accumulate ideas, proofs of concept and vendor proposals but stall when it comes to operational adoption.

The dynamic is familiar in health IT: translating a promising model into day-to-day clinical or administrative use requires data readiness, workflow redesign, oversight structures and staff training, along with clarity on performance measurement and accountability. The result, the report suggests, is a gap between AI experimentation and enterprise-wide impact—particularly when organizations lack a clear strategy for prioritizing use cases or for integrating models into clinical operations.

Federal funding expands for AI-enabled ultrasound and imaging research

On the innovation side, GE HealthCare and the U.S. Biomedical Advanced Research and Development Authority (BARDA) have expanded an agreement aimed at AI-enabled imaging.

reported the deal includes a $35 million expansion focused on AI-enabled imaging capabilities.

While the outlets emphasize different aspects of the initiative, together the reports point to sustained federal interest in using AI to augment imaging—an area where machine learning has been widely explored for tasks such as image reconstruction, prioritization and interpretation support. Federal partnerships of this scale also signal that government agencies view imaging AI not only as a commercial opportunity but as a capability relevant to public health preparedness and healthcare resilience.

AI’s promise in cardiovascular care, and the limits of hype

Beyond imaging, advocates continue to argue that AI could help address major disease burdens. The World Economic Forum highlighted the potential for AI to support prevention and earlier intervention in heart disease, positioning it as a tool that could strengthen screening, risk assessment and care management in the global fight against cardiovascular conditions.

However, the shift from promise to outcomes depends on deployment conditions. For cardiovascular care in particular, AI systems must be trained and validated on representative populations, integrated into clinical workflows, and monitored for performance drift over time. The broader debate is increasingly less about whether AI can generate accurate predictions in controlled settings and more about whether health systems can reliably operationalize tools in real-world environments.

The increased interest from insurers adds a new layer to the AI landscape because payer tools can influence the pace and shape of care delivery, from administrative workflows to utilization management. At the same time, insurer adoption raises questions about transparency and governance: as AI systems are used to streamline decisions, policymakers, providers and patients may demand clearer explanations of how models are deployed and how errors are handled.

Payment remains a pivotal bottleneck for clinical AI

Even where AI tools show promise, a key constraint is how they are paid for. The overall direction is clear: AI is becoming embedded across healthcare’s clinical, administrative and regulatory domains. The more difficult question is how quickly systems can align governance, validation, workflow integration and payment models to ensure that AI tools are deployed safely, effectively and sustainably.
